At CornerstoneVision our goal is to never turn anyone away who is in need of our counseling and psychological services. Our fees are based upon insurance and community

All records are kept confidential and are maintained in locked files. No information concerning a client may be released without the client’s written consent. We observe State and Federal (HIPAA) privacy policies. Please see our web site (www.corvision.org) for more details and for a copy of the appropriate forms to complete.

Service Fees

Fee for service/cash pay rates

(These fees are applicable only when insurance is not being used.)

PhD : $95 per each 45-50 min. session

Masters : $85 per each 45-50 min. session

Church Assistance : $70 per session if not using insurance along with church contract

Premarital Counseling : $70 per session

Adjusted Fee : $90 to $30 (Adjustments are based on family income and number of individuals in the household)

*Report Writing Fees : $135-150 per unit/hour

(Our fees are based upon insurance and community)

Testing/Assessments/Reports : $135-175 per 50 minute unit, depending on procedure codes

Court Services : $195 per 50 minutes-port to port (Retainers Required)

We are contracted with most major insurance companies. Please call for details on contract rates.

Insurance

CornerstoneVision is contracted with several insurance companies (including Medicare / Medicaid) and we can file your claims if we are contracted with your company. Just call the
1-800 number on the back of your insurance card to inquire about outpatient mental health benefits.

Adjustment Fees

For those clients who do not have mental health insurance benefits, CornerstoneVision offers
an adjusted fee scale application based on the number of people in the household and family income. You will need to provide two pay stubs or a copy of
last year’s income tax return when you arrive. For
your convenience you may pay by check, cash, or
credit card.

Church Assistance

It is our desire to provide care through a partnership with the community. Speak with your pastor about the possibility of the church assisting
with the cost of counseling. Have the pastor contact CornerstoneVision to make financial arrangements. Please be assured we will still be able to maintain complete confidentiality between you and
your therapist.
